New Patient Information and Intake Forms

Welcome to Specialty Natural Medicine

Please complete the online new patient documents prior to requesting or booking your new patient appointment, or immediately after booking your appointment.  Failure to complete your new patient forms within 24 hours of your new appointment confirmation will result in your appointment being cancelled.  Any additional appointments booked prior to completing your new patient forms will be removed from the schedule until your new patient forms are complete.

Billing and Insurance Information

We are contracted with the following insurance companies:

Regence, Premera, Blue Cross, Lifewise, First Choice, Uniform and other smaller companies.

As specific insurance plans differ, it is the patients responsibility to verify coverage for the Doctor they will be seeing, as well as for the specific procedures.  Please confirm Naturopathic coverage as some plans will not cover ND services even if our Doctors are listed as preferred providers.

Specialty Natural Medicine can provide the patient with likely procedure codes for typical appointments on request.

There will be charge for appointments that are missed or cancelled within 24 hours of appointment time.
